Interprofessional Primary Care Team (IPCT) Proposals

Dr. Philpott’s Primary Care Action Team has launched a province-wide initiative to ensure every Ontarian is connected to a primary care clinician or team by 2029. Backed by a $2.1 billion investment, the Primary Care Action Plan will expand access through new and enhanced interprofessional primary care teams.

The 2026–2027 Call for Proposals will invest more than $250 million to support the creation or expansion of up to 75 teams, connecting 500,000 more people to care. Ontario Health Teams and their Primary Care Networks will play a key role in coordinating and submitting proposals.

Submission Instructions

Draft proposals are due on Friday, November 7th (end of day) to Ottawa OHT-ÉSO.

Submit your proposal to info@ottawaoht-eso.com.

Ottawa OHT-ÉSO's has brough together a committee to review the proposals, provide feedback, and select those which will move on to be submitted to Ontario Health on November 14th. This committee is comprised of local primary care professionals and client partners who are free from a conflict of interest.

Please note that the scoring matrix is available below. Submitters will also have the opportunity to implement feedback before submission to Ontario Health.
