Primary Care Partner Table
The OHT–ÉSO Primary Care Partner Table will bring a multidisciplinary group of Primary Care Providers to provide leadership to the OHT around engaging with Primary Care Providers in our community, building a trusting relationship between providers and the OHT, and advancing models to evolve a strong foundation of primary health care in health system reforms.
